Is Donald Trump showing signs of incipient dementia? Psychologist Dr Harry Segal weighs in
Clinical psychologist Dr Harry Segal has been closely studying former President Donald Trump’s behavior on his YouTube program Shrinking Trump. In his latest analysis, Segal raised concerns that Trump may be displaying what he described as “incipient dementia” — meaning potential early signs of cognitive decline.
According to Segal, two major factors are driving what he called Trump’s “collapse.” The first is malignant narcissism, a severe form of narcissistic personality disorder marked by a lack of empathy, compulsive lying, and vindictive behavior. The second, which he and fellow psychologist Dr John Gartner have increasingly discussed, is the possibility of cognitive deterioration.
Segal explained: “We’re starting to see some of the early signs of cognitive dissolution on Trump’s part. So we’ve been noting that and chronicling it.”
The Epstein controversy
One issue Segal pointed to was Trump’s handling of the Jeffrey Epstein files. The Trump administration faced criticism for failing to release promised documents related to the late financier. Instead of addressing the matter, Trump has repeatedly dismissed it as a “Democrat hoax.” Segal argued that this type of response reflects not only political deflection but also possible signs of declining reasoning.
He added: “He blamed Obama for it even though Epstein was prosecuted during his own term. Politically and cognitively, that doesn’t make much sense.”
Court losses and “narcissistic wounds”
Segal also suggested that Trump’s recent court defeats, such as rulings against his tariffs and immigration policies, have intensified his psychological struggles. Rather than treating these setbacks as policy disagreements, Segal said Trump reacts to them as personal insults — a pattern consistent with narcissistic traits.
Author Michael Wolff similarly described how Trump reportedly erupts in anger when confronted with limits to his authority, something Segal connected to both fragility and cognitive strain.
Questions about health and fitness
Despite these psychological assessments, it’s important to note that the White House physician has publicly stated Trump is in “excellent cognitive and physical health” following comprehensive medical exams, including neurological testing and a perfect score on the Montreal Cognitive Assessment (MoCA).
Segal, however, concluded that Trump’s behavior raises an open question: “Does he lose political power before he becomes cognitively disorganized?”
